MyChart is a free, online patient portal and mobile app that provides you convenient access to your medical records, as well as many other features to help manage your healthcare. With so many tools offered in MyChart, it can be overwhelming. Here is a quick guide on how to use MyChart.
What is MyChart?
MyChart is an online patient portal to help manage your healthcare. Patients of Lake Charles Memorial Health System can create an account and access it on a computer, tablet, or phone--anytime, anywhere
What can I do on MyChart?
MyChart allows you to:
- View all your health information in one place
- Schedule an appointment
- Check-in online
- Communicate with your doctor
- Refill your prescription
- View test results
- View your After Visit Summary and provider notes
- Access your family members’ medical records
How do I sign up?
There are multiple ways to sign up for MyChart. Each of these paths to enrollment depends on your specific situation.
- The easiest way to use MyChart is to sign up online or by using the MyChart mobile app. Your account will be created by matching your information to your medical record or with a third-party verification.
- Clinical staff can help you sign up directly at the front desk or in the exam room.
- After a visit, you may receive a MyChart activation code via a letter or on a billing statement.
- You may receive a text message or email with an activation code.
I was given an activation code. Now, how do I sign up?
Download the MyChart app on your mobile device or visit lcmh.com/mychart.
Select "Sign Up Now".
Enter your activation code and other personal verification items, such as the last four digits of your social security number and your date of birth. Select Next.
On the next screen, choose the following:
MyChart username. Choose something that others will not be likely to guess but easy for you to remember. It cannot be changed at any time.
Password. Choose a unique combination of numbers and letters, using both uppercase and lowercase letters. Your password must be at least 8 characters and must be different from your MyChart username. Choose a password that you don’t use for other websites.
On the next screen, choose whether you want to receive a notification message in your personal email when there is new information available in your MyChart account. If you opt to receive email alerts, enter your email address.
Can you send me a new activation code if I lost it, let it expire or didn’t receive it?
To receive a new activation code, please call our MyChart Patient Support Center at 337.494.2888 or by emailing lcmh_epic_mychart@lcmh.com.
My activation code does not work. What should I do?
The security of your healthcare information is our highest priority; therefore, your activation code will expire in 45 days. The activation code will also expire after use. If you are unable to use your activation code, please contact our MyChart Patient Support Center by calling 337-494-2888 or email lcmh_epic_mychart@lcmh.com.
If I send a message to my doctor or nurse, when can I expect a reply?
You will generally receive an answer within 1-3 business days. Please note that MyChart should NOT be used for urgent situations. Please contact your medical provider's office for matters that require more timely attention. Dial 911 for a medical emergency.
I have used MyChart at another hospital. How do I link my account together?
If you have been a patient of another healthcare organization or providers office that is outside of Lake Charles Memorial Health System, you may be able to view information from that healthcare organization within your MyChart. This feature is known as "Happy Together". To learn more about the "Happy Together" feature, click here. This feature includes information from other organizations, such as:
- Allergies
- Care Team
- Health issues
- Medication
- Messages
- Test results
- Visits
To view this information, you must link your account. In your MyChart account or mobile app, go to Sharing > Link My Accounts to get started. Then, search for the organization that you would like to connect to and select from the list and click Link Account. After you've linked your accounts, information from the other organization appears in MyChart. You can hover the mouse over the symbol to see where the information is coming from. Can I view a family member’s health record in MyChart?
Yes, you can. MyChart allows easy management for the healthcare of the entire family, with certain permissions. This feature is called Proxy Access. This allows a parent (or guardian) to log into their personal MyChart account and connect to information regarding their family member. To gain access to be proxy to someone else, you must complete a Proxy Consent Form and return it to the medical facility to request access to this convenient service. A patient can have any number of proxies and can be a proxy for any number of other patients.
You may also want to grant a family member or friend access to your medical records when you need assistance managing your appointment and other medical needs. To give someone access to your medical records, grant access via a proxy invites through the Sharing Hub in MyChart.
From Sharing Hub > A Family member, close friend, or caretaker > Manage ongoing access to your MyChart account select the Invite Someone button and enter the name and email address of the family member, close friend or caretaker. This will allow you to give access to someone without help from the clinic. The invited person will receive an email with a link that allows them to set up their proxy access.
