Meet Mohammed Sarwar, MD

Dr. Sarwar is an internist and primary care physician at Lake Charles Memorial Health System. He sees patients at Moss Memorial Health Clinic on Walters Street in Lake Charles.  

A primary care physician is often the first medical professional you see when you're sick. They are specifically trained to act as personal doctors, providing long-term, comprehensive care. Internists see adult patients to diagnose and treat a variety of issues – from colds and flu to managing chronic diseases, like hypertension and diabetes. Primary care physicians, like Dr. Sarwar, are also the access point to specialty care, like cardiology, orthopedics, and surgical services.  

Along with treating a variety of chronic adult diseases, Dr. Sarwar has a particular interest and extensive training in diabetes care. Dr. Sarwar has served as the Director of the Diabetes Clinic and a member of the Diabetic Disease Control Committee at LSU W.O. Moss.  He is certified as a diabetic-preferred provider by the American Diabetic Association and has also been certified by the National Committee for Quality Assurance for achieving quality care in diabetes and has won multiple awards of excellence in diabetes care from Louisiana State University.  

Dr. Sarwar holds a bachelor's degree from Adam Jee Science College and began his medical training at Dow Medical College, where he received distinction for his academic performance in math, physics, biology, and chemistry. He then continued to postgraduate medical training with an internship at the Civil Hospital Dow Medical College and a residency at Brook Dale University Medical Center in Brooklyn, New York.  

He is a fellow of the American College of Physicians, a member of the Louisiana Medical Society, and the Calcasieu Parish Medical Society.  Dr. Sarwar was also awarded the Golden Stethoscope Award in 2023 for providing excellence in patient care, customer service, communication and teamwork.

I'm a new patient. Can I fill out the paperwork before my appointment?
To help prepare for your first appointment, you can download and print new patient paperwork prior to your visit. Please click Download under Patient Forms above. If you are not able to to do this before your visit, you can fill out paperwork in the office. Please note: some clinics may have other specific forms for new patients in addition to the forms above.
It's my first visit, do I need my medical records from my previous doctor(s)?
If you have seen another physician, it is helpful to have his/her previous office records. A consent for the release of information must be signed and then we can request records from your previous physician.
Do you need my insurance information at the time of my visit?
Yes, we require your insurance card at time of service. We will file all claims to our participating insurance groups. Payment of insurance co-pays, deductibles, and all fees NOT covered by your insurance company are expected at the time of service. Fees may be paid by cash, check or credit card.

Our physicians and nurses are with patients throughout the day, so it is usually not possible for them to immediately answer phone calls. We will return your call as soon as possible. I have a question about my bill. Who do I contact?
Your Memorial Medical Group bill includes charges for goods and services rendered by your physician. Depending on your care, you may receive separate bills for services such as X-rays or lab work. You may contact our financial services department at 337.494.3265 with any questions or concerns about your bill or to pay your bill by phone. You may also visit the billing and financial services office located at 3050 Aster Street in Lake Charles. It is open Monday – Friday from 8:00 am to 4:30 pm.
