Online Patient Record Request is the tool used to get a comprehensive paper copy or digital copy of medical records. For continuity of care purposes, use the Online Patient Record Request.Patients can receive their medical records in the following way:
To use the Online Patient Records Request, a copy of the identification of the patient or their legal representative is required. For example, requests for minor's records need identification of legal custodians; caretakes need medical power of attorney identification. Requests for records of a deceased patient require death certificate; request for newborn records require a birth certificate.
Your doctor's office will need to fax the required forms to 337-494-3148.
The medical records office at Lake Charles Memorial Health System does NOT fulfill request for birth or death certificates. For these requests, contact the state Vital Records Office in New Orleans. Birth and death certificates can also be requested in person at the Sulphur Vita Records Office at 1551 E. Napoleon Street in Sulphur, Louisiana. Click here for more information.
Please note, the Louisiana Vital Records Office only maintains birth and death records that occurred in Louisiana.
