Things to Know:
- Admit orders are not needed to transfer a patient from your emergency department to Memorial's Emergency Department. Phone Number: 337.494.3036
- Admit orders from a Memorial physician are needed to transfer a patient from your emergency department to Memorial for an inpatient admission.
- Admit orders from a Memorial physician are needed to transfer a patient from your hospital inpatient admission to a Memorial inpatient admission

- Facilitate the details necessary for transfer, including any paperwork and medical documents that Memorial will need
- Coordinate nursing unit and bed arrangements for the patient
Outside facilities can help ensure patient identification by faxing the patient's demographics or face sheet immediately to 337.494.2956.
